What to Complete Before Your First Visit
We ask every new patient to fill out a short set of forms before their appointment. These cover your health history, current symptoms, insurance information, and treatment consent. Having them in hand before you arrive means your first visit stays focused on you — not on paperwork.

Frequently Asked Questions About New Patient Forms
Can I fill out my chiropractor forms online before my appointment?
At this time, our forms are available as downloadable documents to print and complete at home. Bring your signed copies with you on the day of your visit. This keeps your information within our office rather than routed through an outside platform.
What if I forget to bring my completed forms?
We'll have blank forms available at the office. Arriving 10–15 minutes early on your first visit gives you time to complete them before your appointment begins.
Do I need to fill out forms if I'm a returning patient?
Returning patients generally do not need to complete the full intake paperwork again. If your health history, insurance, or contact information has changed, let us know when you call and we'll note what needs to be updated.
What information will I need to complete the forms?
You'll need your basic contact and insurance details, a summary of your current symptoms and health history, and information about any prior treatments or injuries. Most patients complete the full set in about 10 minutes.
Is there a separate form for auto accident or personal injury patients?
Yes. If your visit is related to an auto accident or personal injury claim, please let us know when you schedule. We'll make sure you have the correct documentation for your case type.
